What Being a Sibling Doula Looks Like: Supporting Big Siblings Through Birth

When a family grows, the arrival of a new baby brings excitement—but it can also bring a lot of uncertainty for the older siblings. Being a sibling doula is all about guiding them through this transition with care, reassurance, and connection. It’s not just about keeping them occupied—it’s about helping them feel included, valued, and safe throughout the birth experience.
Here’s what sibling doula support looks like before, during, and after birth.
Before Birth: Preparing Siblings for the Big Day
Preparation is key. Older siblings often feel a mix of excitement, curiosity, and anxiety. As a sibling doula, my role is to help them understand what to expect and give them tools to express their feelings.
What I do:
Meet With Siblings: Spend time answering questions in an age-appropriate way. What is labor? What will the hospital look like? What will the baby do when they arrive?
Normalize Emotions: Siblings may feel excited, jealous, nervous, or confused. Talking through these feelings helps them process them safely.
Role-Play & Practice: Using dolls or play scenarios, siblings can practice holding the baby, helping mom, or following safety rules.
Create a Personalized Plan: Decide what the sibling will do during labor—will they stay at home, visit briefly, or spend time in a sibling space at the hospital or birth center?
Preparation gives siblings a sense of control and reduces surprises on the big day.
During Birth: Presence, Reassurance, and Engagement
On the day of birth, emotions run high. A sibling doula ensures older children feel safe, seen, and involved—even if they aren’t in the room for the entire labor.
What I do:
Provide Emotional Support: Listen, validate feelings, and help siblings manage nervousness, excitement, or frustration.
Keep Siblings Occupied: Games, drawing, books, or quiet activities help them feel engaged and keep energy calm.
Update in Real-Time: Explain what’s happening in simple terms. For example: “Mom is having strong contractions, and the baby will be here soon!”
Support Parent Connection: Help siblings deliver messages of love or encouragement to the birthing parent, reinforcing their role in the process.
Safety and Supervision: Make sure siblings are safe, comfortable, and have clear boundaries while navigating a busy labor environment.
This presence allows siblings to feel included without overwhelming them—or the birthing parent.
After Birth: Welcoming the New Baby
The moments after birth are full of awe, bonding, and new dynamics. Sibling doula support helps older children navigate these changes with care and curiosity.
What I do:
Guide Introduction: Support a gentle, safe first meeting between sibling and newborn. Encourage touch, curiosity, and connection while respecting both parent and baby’s space.
Normalize Emotions: Siblings may feel excitement, jealousy, or protectiveness. Helping them express these feelings openly supports healthy adjustment.
Provide Special Time: Sometimes siblings need one-on-one attention to feel included in the family changes without competition.
Offer Keepsakes or Rituals: Creating a “big sibling” gift, card, or handprint activity can help them celebrate their new role.
This guidance ensures siblings feel empowered, connected, and valued—not overlooked—during a time when attention is naturally focused on the new baby.
The Heart of Sibling Doula Support
Being a sibling doula is about honoring every family member’s experience. Birth isn’t just for the birthing parent and baby—it affects the whole family. By providing reassurance, guidance, and safe inclusion, sibling doulas help older children feel confident, loved, and proud of their role in welcoming a new life.
